Sustainability and Transition tool

A part of implementing a project is planning for the end of that project. The Transition and Sustainability planning document identifies who will be responsible for the new changes, processes, and ongoing training, and how the transition of those areas will occur.

Sometimes we don’t know what our end -users will need until they need it. Having an implementation support team in place at Go- Live and can increase stakeholder success and in turn lead to faster realization of benefits.

The training team is a great resource at this time, as are subject matter experts who can quickly create job aids and helpful tips for end users.

The Sustainability and Transition Plan provides an approach to maintaining the new processes and achieving a day-to-day cadence of doing business once the change has become operational.
